Position classification

Hybrid -- Job duties and expectations allow for onsite and remote work scheduled every week. Employees in this classification are regularly scheduled onsite one (1) to four (4) days per week based on the County needs and as determined by Department Director. Hybrid classified roles can be onsite more than the set minimum based on employee preference.

 This position will provide ongoing case management services and accurate documentation of safety, permanency, and wellbeing of youth receiving services within the home or in out-of-home placement to include kinship.

 

 Additionally, this social caseworker will complete thorough assessments of clients, including family history and needs and will also provide appropriate recommendations and services for youth ages 10-17 with possibly younger siblings in the home, who are involved with the juvenile delinquency system, experiencing parent child conflict issues and/or abuse and neglect.

 

 This position attends monthly case staffing's, court hearings, and regularly conducts home/site visits and works closely and develops professional relationships with families and community partners, including but not limited to probation, schools, and treatment providers. This worker will professionally represent the agency in the juvenile court system, D & N hearings, with service providers, and in other community settings, including state meetings/workgroups and trainings.


 Work with a goal to have youth in the most family-like/ least restrictive setting that meets their needs, to empower the youth to succeed in their personal goals, build permanent connections, and achieve legal permanence for the youth.  

Examples of Duties for Success

  • Completes thorough assessments of family and youth for individual/community risk, services, and sources.
  • Ensures appropriate treatment needs are being met on a continual basis by various service providers.
  • Documents case activities while complying with all federal and state mandates and guidelines.
  • Participates with interdisciplinary consultation and collaboration on a regular basis.
  • Prepares court documents/reports and provides recommendations and testimony as needed. 
  • Participates in case-specific court hearings, consults with county legal staff as needed, and may provide coverage as needed for teammates at court hearings. 
  • Coordinates and maintains professional communication with collateral agencies.
  • Develops comprehensive and appropriate treatment plans for clients that address therapeutic needs.
  • Supervises visits for clients on caseload when no other resources are available and/or when appropriate.
  • Knowledge of developmental needs of children ages 10 and older, with an understanding of the latency and adolescent ages, to include the IDD population
  • Knowledge of Case Management/Care Coordination experience required.
  • Provides crisis management for case-specific emergencies, including placements and removals of children, as needed, to ensure safety. Provides ongoing safety monitoring and assessment and develops/updates safety plans as required to ensure safety for children on caseload.
  • Participates in all required state and agency trainings on an annual (or as required) basis.
  • Provides face-to-face contact with clients as required by regulatory standards and agency policy (may sometimes include in-state and/or out-of-state travel); communicates with collaterals; and attends case-specific staffing's. Actively participates in Foster Care Reviews, audits as required, and ensures that client charts are up-to-date and accurate.
  • Participates in scheduled supervision, team meetings, section meetings, and division meetings.
  • Provides customer service through timely response to phone calls, e-mails, letters, and other communication. Provides direct problem-solving with clients and collaterals in a professional and courteous manner.

Qualifications for Success

  • Knowledge of Adolescent Development, family dynamics, child abuse and neglect dynamics, mental health, and substance abuse issues; working knowledge of crisis intervention, youth cultures, and juvenile court procedures.
  • Knowledge of and ability to work within the juvenile court system.
  • Skills and sensitivity in working with issues of clients from a variety of cultural, ethnic, and socio-economic backgrounds.
  • Knowledge of various computer systems to complete mandated case management reports and documentation (i.e.: Trails, Records Management, Word, Nexus Lexus, etc).
  • Ability to build professional relationships with various community resources.
  • Ability to prioritize tasks and make informed decisions.
  • Ability to work cohesively with other members of a team.
  • Ability to communicate clearly and concisely, both orally and in writing.
  • Ability to establish, maintain, and foster positive and harmonious working relationships with those contacted in the course of work.

Experience: 

  • One (1) year of full time professional social casework experience, acquired after the degree in a public or private social services agency, in an area appropriate to the position.

Education and Training:

  • Bachelor's Degree in one of the human behavioral science fields.
  • An MSW may substitute for all of the required experience.
  • An educational waiver is available for those individuals who do not meet the education requirements to be a social caseworker and are currently enrolled in a bachelor’s program from a higher education institute.  A bachelor’s degree must be obtained within one (1) year from hire date with related coursework to job duties as outlined in 7.603.1 (click here for more info).  If a degree is not obtained within one (1) year from hire date, employment with the agency may be impacted including, and up to termination.  Employee will be responsible for updating the hiring manager every six (6) months of their education progress.
  • Proof of education, (degree or transcripts), and license (if candidate has received one) are required.

License or Certificate:

  • Colorado Child Welfare Caseworker certification required and must be completed within three (3) months of employment, through the Colorado Training Academy (State Child Welfare). Certification through another State will require proof of completion and may substitute for all or part of the training required by Colorado.

 

  • Possession of a valid driver's License and proof of automotive insurance.

Background Check: 

  • Must pass a criminal background check.
INTERNAL APPLICANTS ONLY: Employees who are currently in a case manager/case-carrying position in Children and Family Services (CFS) may have a 120-day delayed transfer/promotion period prior to moving into any new position in Human Services.

Other: 

  • Ability to work in a standard office environment.
  • Ability to utilize personal vehicle or other mode of transportation to make visits to client homes, travel to court, and other public facilities in all types of weather.
  • Ability to carry large case files.
  • Ability to conduct home visits in unknown and uncontrolled settings which may include animals, feces, airborne contaminants, gang members, weapons, and other environmental hazards.
